Initial Psychiatric Evaluation: $400 / follow up medication management $200 / Therapy follow up to $150
Psycho-Social Assessment: $225.00 Therapy Follow up $150
Video Consultation (Telepsychiatry): $400 / Follow up $200
therapy With LPC/A: $90
Completion of Forms: (Such as FMLA) & Disability: $300 *forms will only be filled out for established patients at the doctors' discretion
Letters to Universities, Employers Etc.: $30
Cancellation With 24 business hours (tuesday-friday) Notice: $65
Cancellation With 48 business hours (tuesday-friday) notice: $50
Telephone calls with providers: $30
*multiple cancellations and no-shows will result in dismissal from practice
